If I click on the video to make it larger, it makes it larger for me, but also for them so they are effectively looking at themselves. Awkward and distracting. One work around is that I can share the program rather than the whole desktop but it isn't ideal if I'm moving between applications.

If this is the case, I'd recommend looking at a Teams Room System endpoint that will display content far end full screen, and should allow you to select video feeds to want to see from your client.
